Your notice must be signed by the copyright holder or their authorized agent, identify the copyrighted work that infringement is claimed on, provide the exact location of the material you claim infringes that copyright (with enough detail that we can easily find and identify the material, not just the name of a user or blog, and what specifically in any post you believe infringes), your name/address/telephone/email, a statement that you have a good faith belief that the material is not authorized by the copyright holder, their agents, or the law, and a statement signed under the pains and penalties of perjury that the notification is accurate and that you are authorized to act on behalf of the owner of an exclusive right you allege is infringed.
